Use this questionnaire for getting started in using OOM - you just need to

Directions:
Print this sheet out or get a piece of paper and pen to take notes.
Fill in the box that best describes the frequency (Rarely, Sometimes, Never, Undecided) you experience each of the
"Signs" under each of the Essential Actions (EA's). Tally your scores for each EA.
Go to the end of this section to see how you may translate your scores in to action
based on using The Organic Organizing Matrix if you would like. At least by
the end, you may just be even more aware of where your blind spots are!

Essential Action (EA) 1

My belongings are spread out all over the place
I start activities without every item I need and have to stop and go get what I need
I waste at least 5-10 minutes per week looking for things
I buy multiple copies of the same product because I cannot easily determine whether I have more
I lose personal items such as: keys, wallet, checkbook, cell phone, purse
I lose all kinds of things such as: paperwork, clothes, addresses, money

Essential Action (EA) 2

It is faster and easier to do it myself than to ask for help
Saying "no" brings up more anxiety than the idea of having too much to do
I say "yes" to doing things without giving myself time to think about it
I feel taken for granted, stuck, or resentful that I seem to have more responsibility than others
When I say no, I find myself explaining my decision, apologizing, or justifying why
I have time for everyone and everything else, and rarely have time for myself

Essential Action (EA) 3

I only have time to handle the most demanding, immediate activities or crises
I feel guilty or silly taking time to pick up, organize or improve my space
There is so much other work to be done, so organizing is at the bottom of my list
I have a hard time "chunking" or breaking projects down into smaller steps
If I can't organize it all at once, at the right time or way, I don't bother at all
I find myself wishing my mess would just disappear

I waste at least 15-20 minutes per week looking for things because they get lost in the shuffle
I have a hard time remembering where I put things
I have a hard time keeping related items together and organized in one place
Stuff stays out when I am not using it because I have a hard time putting it away
I have stuff on the floor because there is no where to put it
I procrastinate or quit when I can't easily find the items I need for activities/projects

I forget to leave time in my schedule for regular ongoing and daily tasks
I have a hard time sticking to my schedule
I forget to leave enough room in my day for the unexpected

I spend 5 minutes or more each day looking for the same things I looked for yesterday
My desk, floor and/ or car are unmanageable and covered with "stuff"
I set something down for one minute and I can't find it, but I know I just put it "there"
I almost run out of things such as gas, cash, food, clean clothes, supplies
I feel overwhelmed by the mess around me
I forget where I left off on projects/tasks and so I avoid getting back to them

I scramble to find everything I need when I am getting ready to go somewhere
I run late for appointments, the airport, events
I keep my to-do list and/or calendar in my head
I forget and miss appointments, important dates, things I said I would do
I can't tell if I am on task or not
I am afraid if I put things away that still need my attention, I will forget about them

I work longer hours than I prefer or intend in order to get things done

I forget how longs things actually take me
I forget to eat, exercise, get enough sleep, breathe, stretch, leave time to transition

Things on my to-do list seem equally important
I dread Mondays, or whatever the first day of the working/school week is
I can't think straight and my decision making ability is strained
I am unclear about which activities and projects need my attention the most
I let seeming crises distract me away from my priorities and goals

What A High Score Means About Any "Essential Action (EA)"
Rarely:
You are most likely succeeding at doing this EA--keep up the good work!
Sometimes:
You are most likely applying this EA part of the time and you could just be
more consistent with it--keep it in your repertoire of EA's you use.
Often:
This would be an EA for you to work on!
Undecided:
You may want to spend time learning more about this EA, or observing yourself
more closely in this area.
